Case Study 1

Transfer from CPAP to HFNC

Patient Description:

  • Ex-preterm
  • 35 weeks corrected
  • On CPAP 5 → 6 → 7cm H2O
  • Escalating oxygen requirement on CPAP

Concern: CPAP interfaces were causing discomfort and increased respiratory support requirement

Measuring the Mean Airway Pressure on a Range of High Flow Rates Allowed Targeting of Pressure

HFNC
4L

Monitor reading 2.78

HFNC
6L

Monitor reading 3.59

HFNC
8L

Monitor reading 6.85

Measurement of pressure generated on a range of high flow rates allowed selection of 6L flow to provide 3-4 cm of PEEP

Following Transfer to HFNC the Oxygen Requirement Gradually Decreased
